The literature on Artemisia annua and the artemisinin-based drugs derived from it is vast. A bibliometric analysis found around 12,985 publications on artemisinin and important derivatives in the Web of Science for 1973–2023. An earlier analysis had already counted 8,466 publications for 2000–2021. A literally complete list would therefore include not only medical studies, but also research on plant breeding, biosynthesis, chemical synthesis, analytics, industrial production, and agronomy.
For this research page, the scope is therefore deliberately kept grounded: medically and biologically relevant studies on Artemisia annua, artemisinin, artesunate, dihydroartemisinin, and artemether are recorded and classified—including positive and negative human studies, animal and cell research, pharmacokinetics, safety, and important reviews. Pure agronomy, industrial active-ingredient production, and total chemical synthesis are not central to this collection.
The strongest clinical evidence concerns the treatment of malaria with standardized artemisinin derivatives, particularly artesunate. In addition, clinical and preclinical research areas include schistosomiasis, cancer, inflammatory and autoimmune diseases, viral infections, other parasites, neurodegeneration, fibrosis, pharmacokinetics, the plant matrix, and safety. It is crucial not to equate these areas with one another: Clear clinical benefit from artesunate in severe malaria says nothing about the effectiveness of an Artemisia annua preparation for another disease.
Artemisia annua is a complex plant containing numerous constituents. Artemisinin is a single characteristic natural compound from it; artesunate, dihydroartemisinin, and artemether are pharmacologically defined derivatives. Preclinical studies show that the plant matrix, extraction method, and accompanying compounds can influence release, absorption, and metabolism. However, this does not automatically mean that a whole-plant preparation is more effective or safer in humans than a standardized drug.
The large randomized SEAQUAMAT and AQUAMAT trials, along with systematic reviews, are part of the reference evidence base for artesunate research. In severe malaria, parenteral artesunate reduced mortality compared with quinine. Studies of traditional Artemisia annua preparations must be clearly distinguished from this: They showed some initial antiparasitic effects, but do not provide a sufficient basis for using plant preparations as a substitute for established malaria treatment.
Artesunate and combinations with other antiparasitic agents have been investigated in several human studies. Results depend on the regimen and population; alongside positive signals, there are also disappointing or negative findings.
Cancer research is broad, but still clinically limited. There are small human studies or early clinical programs involving colorectal cancer, lung cancer, breast cancer, solid tumors, cervical precancers, and individual case series. By far the larger body of literature consists of cell and animal models investigating apoptosis, ferroptosis, angiogenesis, DNA repair, treatment resistance, and combinations with chemotherapy or radiotherapy.
In addition to a randomized Artemisia annua study in rheumatoid arthritis and a small osteoarthritis pilot study, extensive preclinical research exists on lupus, psoriasis, experimental autoimmune encephalomyelitis, asthma, and inflammatory bowel diseases.
Artesunate has been clinically investigated against CMV, among other conditions, in small case series. For SARS-CoV-2, Toxoplasma, Babesia, and Leishmania, the evidence consists predominantly of preclinical studies. The levels of evidence differ substantially.
Artemisinin exhibits distinctive time-dependent pharmacokinetics with autoinduction of metabolism. Delayed hemolysis after intravenous treatment of severe malaria has been documented for artesunate, among other effects. Pregnancy and neuro-/ototoxicity data must be assessed in the relevant context of use.
Several animal and in vitro studies compare whole-plant material or Artemisia annua extracts with isolated artemisinin. Differences in bioavailability, CYP enzyme interactions, and antiplasmodial activity have been observed. These studies are scientifically interesting but remain preclinical. Even two studies investigating “Artemisia annua extract” need not use the same material: genetics, location, climate, harvest time, plant part, drying, and extraction methods can substantially alter the chemical profile.
